Legends of Stonehaven is a medieval and fantasy RPG server running Season 2. It is based on a Fantasy Medieval RPG modpack and it launched on Thursday 19th March.
Combat is heavily revamped with new animations and a dodge mechanic. You also get an in depth skill tree and origins so your character build can feel different from other players. There are custom weapons armor trinkets bosses origins and divine blessings that are balanced for this pack.
The world is made for exploring since the overworld nether and end generation are overhauled. There are hundreds of new dungeons and structures plus 5 new dimensions. Enemies also scale up as you go further out so it stays challenging.
There are over 200 quests you can do solo or with friends and some add story and lore. A quest shop lets you spend coins on useful items. The server also cares about performance and has lots of QoL stuff like backpacks and storage systems.

MineScape is a very early alpha server that is all about late medieval faction warfare with RPG style progression and a player run economy.
You can go full PvP and fight for your faction or you can grind like an MMO. You can also play more solo as a trader gatherer craftsman or contractor who sells gear and builds stuff for other players.
It has custom crops plus custom weapons and armor with special effects. The world also has stronger than vanilla mobs and boss mobs that are meant for groups.
Trading is a big deal here and it is made safer with a secure MMO style trade window so you do not have to drop trade. Everyone spawns in the main market hub called Osthaven where buying and selling happens.
You can set up temporary wilderness camps for travel hunting and gathering and they are protected with WorldGuard. Since it is early alpha they want testers to find bugs check balance and give honest feedback to help shape the server.

I found a big modded Minecraft server built around That Time I Got Reincarnated as a Slime. The goal is to recreate the Tensura world with a magitech vibe like Tempest. You start in the Capital City of Tempest and it was built by the community. There are Tensura characters walking around as NPCs and the screenshot shows stuff like Rimuru and Luminous and Testarossa and Ultima and Carrera as custom entities.
It runs on 1.19.2 and it is huge with 300 plus mods. There are over 2000 quests and a crazy amount of skills with their own quests to unlock them. You can pick from over 50 races and there are 500 plus evolutions plus custom dimensions. It also works like a standalone RPG even if you do not know the anime.
The main questlines are fully covered for Vanilla Minecraft and Minecolonies and Ice and Fire and Apotheosis and Create. The quests explain a lot so you do not have to alt tab all the time. There is a full economy system with shops and currency converting and skill mastering. There is also an Adventurers Guild where you start at F rank and work up to SS class by fighting monsters and stronger stuff.

This is a medieval themed modded server on Java 1.12.2. The main idea is building kingdoms and fighting long wars with big sieges.
You can make npc soldiers and workers and you can use siege machines like catapults. People already started forming a few kingdoms since the server began last week.
There is claim protection but it gets disabled during a siege in wars. The server is also pushing more lore and rp and there is a backstory but you do not have to roleplay if you do not want to.
A lot of the focus is on building nice towns and forts with furniture and extra building blocks. There are also other mods that add stuff to do besides war.
If you want action without fighting players there are pve structures and bosses. They also plan pve events soon.
